I have possessed a specially protected raptor or other bird of prey for two consecutive years within the last five years in another jurisdiction prior to today's date as demonstrated by the attached documents (copies).

I have previously held a General Falconry License or Commercial Falconry License within the last five years prior to today's date, as demonstrated by the attached license or signed affidavit.
