Bait Harvest Area Application

Instructions
Complete one application form for each BHA.
Please enter the appropriate scores into the boxes indicated (fill out questions under section A or B, and section C) and bring the completed
application to the Ministry of Natural Resources office where the BHA that you are applying for is located. Please bring supporting
documentation so that your score can be evaluated by Ministry staff.

Applicant Information

Section A - For Commercial Bait Harvester Licence Holders Only

1. Do you hold a valid Commercial Bait Harvest Licence for this year?

If Yes, score 5 points.

Number of years

Score 1 point for each year you held a licence

3. If No to question 1, have you held a commercial bait harvest licence within the last 3 years?

If Yes, score 1 point.

4. If No to questions 1 and 3, have you been a designate on a Commercial Bait Harvest Licence within the past five years?

If Yes, score 1 point for each year up to a maximum of 3 points.

5. Answer only one of the following questions:

a) Do you hold a Bait Harvest Licence for a BHA which shares a common boundary with the BHA applied for?

If Yes, score 5 points.

b) Do you hold a Bait Harvest Licence for a BHA where the boundaries touch on corner or are located less than a kilometre from the BHA applied for?

If Yes, score 3 points.

c) Do you hold a Bait Harvest Licence for a BHA within 100 kms of the BHA applied for?

If Yes, score 1 point.

6. Are you already licensed for leeches on the BHA applied for?

If Yes, score 5 points.

Section B - For Commercial Bait Dealer Licence Holders Only

1. Do you currently hold (or have previously held) a valid Commercial Licence to Deal in Bait?

If Yes, score 1 point for each year you held the licence up to a maximum of 5
points.

Distance to BHA (km)

In the vacant BHA, score 5 points. Within 100 km of the BHA, score 3 points. Further than 100 km, score 0 points.

Have you been a designate on a Commercial Bait Harvest Licence within the past five years?

If Yes, score 1 point for each year up to a maximum of 3 points.

Section C - All Applicants Must Answer the Following Question

Have you been convicted of any bait related offences by the Ministry of Natural Resources and Forestry in the past 3 years?

Administrative - Deduct 2 points for each conviction
Conservation - Deduct 5 points for each conviction
